PROGRAM INFORMATION |

Fill out the form. Request an application in person or call 972-941-7335 to have it mailed. Geriatric Wellness Center, 401 West 16th, Suite 600, Plano, TX 75075. The Geriatric Wellness Center is located in the Plano Senior Recreation Center.
Turn in the form. Return the form by mail or in person to the Geriatric Wellness Center (address above)
Wait for a call. You will be called by a staff member from the Geriatric Wellness Center for a quick assessment and to inform you if you have qualified. |

Plano Senior Rides is a program for those Plano residents age 65 and older who lack adequate access to transportation services. Plano Senior Rides is comprised of two different options:
Mileage Reimbursement. Participants receive a $.50 per mile reimbursement for car trips when a non-residing friend or family member drives them. The participant chooses their own driver.
Taxi Voucher. Participants request voucher booklets for $2.50 co-pay for a booklet valued at $10. The vouchers can then be applied toward the taxi fare. |

HOW DO I QUALIFY? |

You must FIRST be qualified before requesting reimbursement or calling a taxi.
This program is administered by the Geriatric Wellness Center of Collin County and funded by the City of Plano. |
